TEAM Rosters (use spreadsheet below):
Rosters are due 1 week before any conference game to be played if games are scheduled before the deadline dates.
Must use this Official Roster Spreadsheet. You must make sure you keep it in .xls format. This spreadsheet can be used for all TEAM sports. See the sample instructions tab and then enter your players based on the division/gender tabs shown at the bottom of the spreadsheet.
Download the excel spreadsheet. Fill each tab out as needed and then email to email@example.com .
Each player and parent must read and sign it annually !!
Please download and make copies for all participants.
Changes that started in 2019
Please make sure you enter phone number in next to last column.
REQUIRED - Concussion column. Indicate if player & parent (guardian) signed the form and turned it in. This will be the last column.
If you use the spreadsheet from 2018 - please add these 2 columns at the end of the old spreadsheet before submitting. Phone column, then concussion column.
Changes to spreadsheet starting Fall 2020.
For Age and Gender verification - the original birth certificate must be looked at. For the gender column enter 'F' or 'M' in the spreadsheet column instead of X, and for the age column, enter Yes or No. For the coaches, they need to read all the rules on the website and the pdf attachment of rules. A date must be provided that they have done so. Remember coaches must annually take the concussion training. Click here for pdf rules and concussion training link.
Note. If using a prior spreadsheet from last year please make note of the 2020 changes needed before submitting the roster.
Please BE SURE THAT A NON-RECRUIT / NON-TRANSFER FORM (NR/NT) HAS BEEN SIGNED FOR EACH PLAYER. PARENTS/GUARDIANS CAN DO THIS ONLINE or YOU CAN DOWNLOAD AND PRINT A HARDCOPY (SEE RIGHT) FOR THEM TO SIGN FOR YOUR RECORDS.
(Note. Mainly applies if the parent/guardian has never done one in a prior registration for this child or another child, but you can have them to acknowledge it again.)
